Join the Transitions Clinic Network

The Transitions Clinic Network (TCN) is a national network of communities that provide health services to recently released prisoners. Funded by the Jacob and Valeria Langeloth Foundation, the TCN is a free opportunity for professionals caring for medically complex returning prisoners to communicate with their peers and learn best practices.

As state budgets dwindle, more and more prisoners with chronic diseases, mental health and substance use diagnoses are returning to communities ill-prepared to address these individuals’ complex health needs. The TCN is a free service that allows professionals to network and learn from each other. As a member of the TCN, community partners will receive free technical assistance and consultation in their efforts to design and implement tailored healthcare services for returning prisoners. The TCN will provide free trainings for medical providers and other members of health care teams including peer advocates, patient navigators and community health workers; and will also offer evaluation support and training in community-based participatory research methods. Stipends for health team members and travel may be included.

TCN staff include the founders and directors of the nationally renowned Transitions Clinic based in San Francisco, as well as highly trained physicians, community health workers and researchers with years of experience caring for incarcerated and post-incarcerated populations.
